S & P 이식법 (STICK-AND-PLACE TECHNIQUE)

슬릿 이식법 중 집도의가 이식 홀을 만드는 즉시 이식 전문 간호사가 슬릿 홀로 모낭을 이식하는 방법입니다.

 

집도의는 sticker 역할을 하고 전문 간호사가 placer 역할을 합니다.

S&P 이식법을 시행하기 위해서는

① 잘 다듬어진 적당한 모낭을 사용합니다.(Use Properly trimmed and Shaped Grafts)

② 슬릿 홀과 모낭의 크기가 맞아야 합니다.(Match Incision Size to Graft Size)

③ 프리즘 루페 확대경을 사용합니다.(Use True Prism Loupes)

④ 다양한 이식 경로를 계획합니다.(Plan on Multiple Passes)

⑤ 모낭의 기저부를 먼저 적당히 이식해야 합니다.(Proper Initial Insertion of the Graft Base)

⑥ 한 수술시야에 이식 모낭과 슬릿이 들어와야 합니다.(Keep Grafts and Incisions in the Same Field of View)

⑦ 모낭을 같은 방향별 그룹으로 세분화하여야 이식 속도를 높일 수 있습니다.(Organize Grafts to Increase Speed - Group of 20, facing the same direction)

⑧ 시계추처럼 리듬을 탑니다.(Develop a Rhythm (“like a clock”))

 

S & P 의 장점

① 적은 출혈 (Less Bleeding)

② 작은 슬릿 홀과 적은 혈관 손상 (Smaller Incisions/Less Vascular trauma)

③ 슬릿 홀을 찾기 쉽고 정확한 각도와 방향에 따라 이식 가능 (Easier to Identity and Follow the Exact Angle and Direction of Incision)

④ 적당한 슬릿 홀 크기 (Ability to Adjust Incision Size)

⑤ 피로감이 덜한 이식 (More relaxed Procedure for the Assistant - less stressful and eye fatigue to assistant)

 

S & P 의 단점

① 오랜 이식 시간 (Greater demand on physical time – only physician’s job)

② 2명만이 작업 (Only one team placing at a time)

③ 익숙해지기까지 배우는 시간이 김 (Long learning curve to achieve speed – 1~2 years, +800 S&P/hr)

④ 전체적 패턴, 분배 조절의 어려움 (Less control over pattern and distribution)
