모발이식 국소마취제의 부작용

1) 전신증상 (Systemic Toxicity)

 

국소마취제의 전신적 혈중 level은 농도 의존성 축적의 효과로 치료효능에서 독성효과까지 나타나며 이것은 국소마취제의 역가(potency), 총주입량, 전신적 흡수 속도, 약물 생체이용률(drug bioavailability), 약물의 제거속도와 관련이 있습니다.

 

독성은 주로 중추신경과 심혈관계에 작용하고, 가장 먼저 중추신경계 증상이 유발됩니다.

 

 

 

① 초기 신경증상 (Early neurotoxic symptoms) : 가벼운 두통(light-headedness), 입주변 저림증상(perioral numbness), 쇳내(metallic taste), 시각장애(visual disturbance), 귀울림-이명(tinnitus), 근연축(muscle twitching), 진전(tremors).

 

② 후기 신경증상(Late neurological sequalae) : 발작(seizures), 기면(somnolence), 의식저하(a depressed level of consciousness)

 

 

2) 알레르기 반응 (Allergic Reaction)

 

미주신경성 event와 allergic reaction이 혼돈되기도 하지만 미주신경성 반응은 호흡기 증상과 피부반응이 없는 점에서 구별이 됩니다.

 

알레르기 반응에서 allergen은 ester 마취제의 대사물인 para-aminobenzoic acid와 amide 마취제의 방부제 성분인 methylparaben입니다.

 

anaphylaxis가 생길 경우 환자 monitoring과, 정맥혈관을 확보하고 BLS(Basic Life Support)/ACLS(Advanced Cardiac Life Support) 지침에 따라 처치를 하게 됩니다.

 

일반적으로 epinephrine, steroid, histamine antagonists를 주사하게 됩니다.
