모발이식시 tumescent의 안정성

Liposuction에서 리도카인 사용량은 toxic daily dose의 5배가 넘지만 안전상의 문제는 없었습니다.

이를 바탕으로한 theoretical pharmacodynamics considerations은

1) diluted solutions have slower systemic absorptions

2) increased vasoconstriction slows absorption

3) lidocaine is lipophilic and sequestered in adipose tissue

4) adipose tissue is removed during the operation

5) adipose tissue has a limited vascular supply, thereby moderating drug redistribution from local to systemic 

   circulation.

 

 Modified tumescent anesthetic technique 으로 리도카인의 daily toxic dose보다 적은 양으로  마취실패도 적었으며, parenteral analgesia와 sedative hypnotics 없이 환자의 불편함을 감소시킬 수 있습니다.

 

또한 수술 후 얼굴부종의 빈도도 늘어나지 않았습니다.

 

Anesthetic solutions

1% lidocaine with 1:100,000 EPI (0.5ml of 1:1,000 EPI + 50ml of 1% lidocaine)

NS with 1:100,000 EPI (0.5ml of 1:1,000 EPI + 50ml of warmed NS)

Buffered solution (0.5ml of 1:1,000 EPI + 50ml of 1% lidocaine + 1ml of 8.4% NaHCO3)

 

모발이식에서 tumescent의 도입은 일반적으로 수술시 쓰는 국소마취제의 용량증가에 대한 대안이 될수 있습니다.

환자의 안락함, 마취시간의 연장, 수술 중 지혈(hemostatis)의 측면에서 장점이 있습니다.
